The staff consists of volunteer health care providers from the surrounding community, including:


  • Physicians
  • Nurse practitioners
  • Registered nurses
  • Volunteer receptionist and data entry



Assistance with medication

costs is available.

Patient Eligibility

Clinic is well-connected with referral resources for dental care, vision testing and glasses, physical therapy, and specialty clinics for cardiology, orthopedic care and selected other clinics.  Referral for navigator assistance in signing up for medical insurance available as well.


Services Offered

We provide primary healthcare, including preventive services and, when appropriate, psycho-social and spiritual needs:


  • Monitor the health status of clients
  • Health education classes for persons with chronic illnesses
  • Counseling (including depression, anxiety)
  • Reduction in the severity of illness and disability
  • Equip persons to make healthy lifestyle choices
  • Improve healthcare quality, access, continuity and accountability
  • No medications will be dispensed at the clinic site.  Prescriptions are provided and assistance with medications is available.
  • Medical records and personal information will be confidential and the clients will be treated with dignity and respect. A social worker is available to assist in finding resources in the community.
